Providing clinical case management services for up to 1 year following injury

  • Immediately following a violent injury, clients will receive individualized case management support services such as:

    • Transportation to medical appointments

    • Housing support

    • Food resources

    • Hygiene products

  • Our case managers travel to clients’ homes and communities to deliver sensitive, confidential counseling services

  • Case managers work with clients to set personalized goals that will support their future endeavors after program completion.

    Common goal types include:

    • Employment

    • Education

    • Positive Relationships

    • Housing

    • Mental and Physical Health

  • To deliver wraparound, comprehensive care, case managers make individualized referrals to our community partners based on clients’ needs

Who we serve

If you meet the following criteria, you are eligible to receive FREE care:

Age 8-30

*

Experienced a violence-related injury within the past year

*

Received care from St. Louis Children’s Hospital or Barnes Jewish Hospital for injury

*

Live in St. Louis City or County

(or Illinois within a certain radius)
